Clear, accurate, and timely communication is essential for an excellent patient experience, better outcomes, and enhanced efficiency. But keeping in close contact with patients outside of the care setting is a challenge for many busy physicians, and the orthopedic surgeons at Health First Medical Group were no exception.

Health First wanted to tighten up acute health episode management and begin daily, one-on-one dialogue with patients once they left the hospital. The organization hoped that enabling meaningful, two-way communication and delivering timely, actionable information to patients would elevate patient satisfaction and “Likelihood to Recommend.”

Health First also needed a better method of capturing patient reported outcome measures (PROMs). Their previous data collection tool was cumbersome to administer and took valuable clinic time away from patients and providers. As a result, the office had been capturing less than 20 percent of baseline PROM data.
